Double Weapon not outputting correct second damage on single line display

Greetings,

PCGEN 5.16.4, RSRD+homebrew, fantasy\pdf\csheet_fantasy_simple_grey.xslt (PDF Simple) output
Just added a Dwarven Urgrosh, and an Axe (Orc Double) onto a dwarf and half orc respectively, equipped as a double weapon

When output, I get the expected 3 weapon entries, one double and another for each head seperately.

The double entry lists two attack ratings, that are the same, and both are 8 less than the base attack.

Whereas fighting with a double weapon, I should be at -4/-8 (no feats applicable to either character) ?

5th level Dwarven paladin, str 17, is at +8 for each of the single head attacks, but is +0/+0 for the double, that should be +4/+0 ?
5th Level Half orc barbarian, str 21 (raging), is at +10 for each of the single heads, and +2/+2 for the double, that should be +6/+2 ?

Is this a bug or have I missed something?

Viewing the troubleshooting character:

Hi,

Simple? Change to Standard and see if that fixes your issue. Simple is meant for the characters that
ARE NOT using TWF or Double Weapons.

I did try the standard output, and have equipped the weapon as a double.

For clarity, I retested by loading just the 3.5 RSRD core, created a dwarven fighter (level 1, all stats 14 before being mod'ed for dwarf) and have:
MELEE +3 (BAB1 + 2 STR)

Urgrosh, Dwarven
-5;-5 1d8+2/+1

Urgrosh, Dwarven (Head 1 only)
2w-P-(OL) -1 / 1d8+2

Urgrosh, Dwarven (Head 2 only)
2w-OH -5 / 1d6+1

As I read the two weapon fighting chart (offhand light) PHB P.160, the modifiers should be -4/-8, giving me -1/-5, so the seperate heads tables appear correct, but the combined (double?) chart showing -5;-5 is wrong?

Also, have I missed something on the damage? The second head is doing a point less damage? Half strength mod for a second weapon? Not to mention the double weapon listing shows 1d8+2/+1 rather than 1d8+2/1d6+1 anyway...

I'm able to reproduce the second head damage not displaying on the second line, everything else appears correct for this issue.

Off-Hand Damage is Light, ergo Half Strength for damage is correct.
Numbers for attacks is also correct.
